Businesses may create their own forms as long as they follow the department's guidelines and specifications. Most forms can be reproduced, with a few exceptions (some multi-part forms with assigned numbers, i.e. ST-556). You must receive approval before submitting the form(s) you create.
Please review Guidelines for Substitute and Reproduced Tax Forms. This includes all the information you will need to know and where to send your forms for approval. You will need to review the Specifications for scannable forms (i.e. IL-941, ST-1 - with scan lines). To obtain approval for scannable forms you will need to submit 5 samples with test data. Please ensure the scan lines are exact in their placement to expedite the approval process.
