Businesses may create their own forms as long as they follow the department's guidelines and specifications. Most forms can be reproduced, with a few exceptions (some multi-part forms with assigned numbers, i.e.
ST-556). You must receive approval before submitting the form(s) you create.

. This includes all the information you will need to know and where to send your forms for approval. You will need to review the Specifications for scannable forms (i.e.
IL-941, ST-1 - with scan lines). To obtain approval for scannable forms you will need to submit 5 samples with test data. Please ensure the scan lines are exact in their placement to expedite the approval process.
