On what form do I report sales and use tax and how do I obtain these forms?
We send personalized Forms ST-1/ST-2 to registered businesses. If you are not receiving your pre-printed forms, please confirm your mailing address. If you have misplaced your forms, please email us by using the "Submit a question" form at the bottom of this page, or call us at (217) 785-3707 so that we can send you replacements. It takes approximately 7 – 10 days to receive these forms. If your request is not made in time to allow you to receive replacement returns before the due date, we will fax you a blank form.
