The REG-3-D is a request for missing signatures. If you received the REG-3-D with a Request for Information letter, then the department is missing one or more signatures needed to complete your account. You may also receive the REG-3-D if you registered online using the Illinois Business Gateway as a third party. Please fill out the form in its entirety and return it by mail or fax to the address or number shown on the letter.
