I have changed organization entity types. How do I change this with IDOR?

I have changed organization entity types (e.g., from a sole proprietorship to a corporation).  How do I change this with the Illinois Department of Revenue?

When your business changes organization entity types and is required to get a new Federal employer identification number (FEIN), then you must re-register with the Illinois Department of Revenue.  You will be assigned a new Illinois Business Tax (IBT) number.  At this time, you can not make this type of change using our online registration system.  You must file a new REG-1, Illinois Business Registration Application, indicating the organizational change.
