I received an IDOR-50-MI or IDOR-50-REG. What information is missing on my account?

I received an IDOR-50-MI or IDOR-50-REG letter that indicates the department is missing information for my account. What information is missing?

If you received an IDOR-50-MI letter, then we have found missing information on your Form REG-1, Illinois Business Registration Application. Please verify the information on the back of the letter and completely fill out the listed schedules as thoroughly as possible and mail them to the address instructed.

If you received an IDOR-50-REG letter, then the Department has received a payment or return from you prior to you registering your business. Please fill out the application and any applicable schedules for your business as thoroughly as possible and mail them to the address instructed. If you believe that you received the letter in error, please call our Central Registration Division at 217 785-3707, or you may contact us via email by using the Submit a question form at the bottom of this page.
