How do I make a change to the banking information I have previously registered for electronic funds transfer (EFT)?

Complete a new Form EFT-1, Authorization Agreement for Electronic Funds Transfer, and make sure you mark the box for changes at the top of the form and write the date the change is effective.  For quickest service, you may fax your completed form to us at 217 524-8282.
