How do I change the business account information I originally reported on Form EFT-1, Authorization Agreement for Electronic Funds Transfer?

You must report all changes in business account information (e.g., name, address, telephone number, or contact person) to us so that your account remains current.

To report changes in business account information, please contact our Central Registration Division by phone at 217 785-3707 or via email by using the "Submit a question" form at the bottom of this page.  Please be sure to include your Illinois business tax (IBT) number, business name, and a detailed description of the maintenance requested.
