What do I do if I am a victim of identity theft?

If someone has misused your Social Security number or other personal information to create tax problems for you, the Illinois Department of Revenue cannot resolve these problems.  You must take certain steps to resolve the problem and provide information to us.  Generally, we base our determination on the Internal Revenue Service's (IRS) review of your claim.

Therefore, you must contact the IRS (or a Tax Advocate) at 1 800 908-4490 to file a claim with that agency.  You must send us a copy of the final determination letter from the IRS that verifies your claim and any corrective actions you have taken. 

You should contact your local police agency, file a report, and submit a copy of the report to us.

You also should monitor your credit report and contact any credit agencies if you suspect theft. 

You must also submit a copy of your Social Security card, a state issued photo ID, and any additional documents or explanations in writing.  Please send all information that will help us review your claim to:
