This form is not available to print. The surcharge is due annually on January 20 to cover the preceding calendar year. Your first return, reporting your surcharge due for calendar year 2013, will be due January 20, 2014.
If you close your facility during the year, you must file and pay Form LA-1, Live Adult Entertainment Facility Surcharge Return, within one month of the date the facility closes. If you are required to file Form LA-1 now and cannot access MyTax Illinois, call us at 217 785-6602.
